Skilled Nursing Facility participants located in Connecticut, Maryland, Pennsylvania and Tennessee attended the program in preparation for CMS’ post-acute care value-based care management program which begins in October 2019.
Healthcare payers such as CMS and providers focus on providing the highest value medical care: quality, satisfaction and cost of care for patients/members in the community. Today, a significant percentage of high intensity care is delivered in the post-acute care setting after discharge from the hospital and this trend is expected to increase, particularly for those patients who have complex healthcare needs. That’s why Kairos Health Systems and Value Care Alliance worked together to provide this program to prepare skilled nursing facilities to successfully meet the requirements of the PDPM program which promotes meeting the increasing healthcare needs of patients with complex medical needs and providing the highest quality, satisfaction and cost standards of care.
The HCPro PDPM Boot Camp provided organizations with a comprehensive review of the service requirements necessary to prepare them for new higher acuity post-acute care delivery models and develop a plan to prepare for participation with PDPM, bundled payments and other payer value-based care requirements.
