Lancaster, PA – Kairos Health Systems http://www.kairoshealthsystems.com/ worked with Value Care Alliance (VCA) http://valuecarealliance.com/ recently to promote a PDPM Boot Camp, presented by Stephanie Corbett, DHA, HCPro http://www.hcpro.com/. Skilled Nursing Facility participants located in Connecticut, Maryland, Pennsylvania and Tennessee attended the program in preparation for CMS’ post-acute care value-based care management program which begins in October 2019.
Healthcare payers such as CMS and providers focus on providing the highest value medical care: quality, satisfaction and cost of care for patients/members in the community. Today, a significant percentage of high intensity care is delivered in the post-acute care setting after discharge from the hospital and this trend is expected to increase, particularly for those patients who have complex healthcare needs. That’s why Kairos Health Systems and Value Care Alliance worked together to provide this program to prepare skilled nursing facilities to successfully meet the requirements of the PDPM program which promotes meeting the increasing healthcare needs of patients with complex medical needs and providing the highest quality, satisfaction and cost standards of care.
The HCPro PDPM Boot Camp provided organizations with a comprehensive review of the service requirements necessary to prepare them for new higher acuity post-acute care delivery models and develop a plan to prepare for participation with PDPM, bundled payments and other payer value-based care requirements. Kairos’ and Value Care Alliance (VCA) are committed to help meet the present and future clinical value requirements of skilled nursing organizations in this rapidly evolving healthcare environment.
“We are proud to work with VCA to bring a program like this to their post-acute care network and our larger membership to help these providers meet the demand for high intensity, short term skilled nursing care in the community, “said Jennifer Wimer President/CEO, Kairos Health Systems, Inc. (www.kairoshealthsystems.com). “Skilled nursing facilities who are focused on providing value-based care help patients and caregivers receive the health care services and support necessary so they obtain the right care at the right time in the right setting after discharge from the hospital. They also help to control costs. Skilled nursing facilities are an important provider of medical care that helps patients and their caregivers successfully transition from hospital to home.”
Kairos worked with Value Care Alliance (VCA) to provide the HCPro PDPM Boot Camp for their post-acute care partners. “VCA was excited to work with Kairos to help prepare post-acute care providers deliver high quality, satisfaction and cost effective care and ultimately better serve the patients in our healthcare community,” said Jeanne O’Brien, CEO/President, Value Care Alliance (www.valuecarealliance.com). Kairos Health Systems and Value Care Alliance worked together to provide this program to help prepare post-acute care organizations so that they are well-positioned to succeed in meeting Triple AIM goals.”

About Value Care Alliance
The VCA network covers a broad geography across the state, offering access through its hospital members and their affiliated physician offices and urgent care centers. The VCA has established an advanced care delivery network that has been actively engaged in value-based payment models since 2013. VCA member hospitals and physicians were one of the first Accountable Care Organization cohorts in the Medicare Shared Savings Program. VCA has over 130,000 lives under contract through its participation in value-based arrangements with Medicare, Medicare Advantage, PCMH+, Medicaid, and commercial insurers. VCA’s proven track record improving the health of its members and driving efficiencies in care delivery through its substantial care delivery infrastructure have distinguished it among other provider groups and prompted the Centers for Medicare and Medicaid Innovation (CMMI) to select VCA as one of only 20 Alignment Track grant recipients for its Accountable Health Communities Model.
